Position Summary

The Value-Based Coder position at Dignity Health Managed Services Organization (DHMSO) is a remote role supporting providers and clinical networks in improving quality and risk adjustment coding performance.

As part of the Quality Management/Risk Team, the Value-Based Coder helps identify opportunities for improved documentation and accurate risk-adjusted condition capture. This role plays a key part in process improvement, provider education, and supporting the organization’s value-based care goals.

Key Responsibilities

  • Review patient medical records using population health tools to identify and monitor risk adjustment coding opportunities.

  • Ensure chronic and major medical conditions are accurately coded, while guiding providers on documentation improvement.

  • Analyze clinical documentation patterns and trends to align with CMS Risk Adjustment guidelines.

  • Develop and deliver education materials and tools to improve Hierarchical Condition Category (HCC) documentation accuracy.

  • Participate in performance improvement initiatives across the network.

  • Safeguard patient information in compliance with HIPAA and organizational confidentiality policies.

Minimum Qualifications

  • Associate degree or equivalent work experience.

  • Certification: CPC, CCS, CCS-P, or RHIT.

  • Advanced knowledge of CPT and ICD-10 coding.

  • Strong understanding of CMS HCC Risk Adjustment and data validation requirements.

  • Excellent organizational, communication, and interpersonal skills.

  • Ability to work independently and manage multiple priorities.

  • Proficient in MS Office (Outlook, Excel, Word) and medical record management systems.

  • Strong attention to detail and documentation accuracy.

Preferred Qualifications

  • 2–3 years of experience in risk adjustment or HCC coding.

  • CRC certification (Certified Risk Coder) preferred.
