Position Summary
As a Medical Coder, you will ensure accurate coding of diagnoses and procedures using ICD-10, CPT, and modifier codes, contributing to compliant billing and optimized revenue cycle operations. Coders may work in outpatient or physician billing environments based on experience.
Responsibilities

Review and assign appropriate diagnosis and procedural codes (including HCC coding for risk adjustment)

Utilize encoder systems for CPT, APC, and EAPG coding
-
Review Explanations of Benefits (EOBs), ADT fields, and documentation for medical necessity
-
Identify and validate account edits or discrepancies for resolution
-
Perform data abstraction for billing and quality reporting

Communicate with insurance carriers, billing teams, and providers regarding coding accuracy
-
Maintain daily productivity reports and meet departmental quality standards
-
Participate in ongoing training to stay current on regulations and best practices

Conduct internal audits and coding reviews with healthcare staff
Required Education & Certifications
-
High School Diploma or GED required
-
One of the following certifications (based on role):
-
Outpatient/Hospital Billing: CCS, CCA, or CPC
-
Physician Billing: CPC, CPMA, CRC, CCS, or CCSP
-
General HIM Roles: RHIT, RHIA, CCS, or CCA
-
Experience
-
For Hospital Coder: 1 year of outpatient hospital coding experience
-
For Physician Billing Coder: 1 year of procedural/office coding experience or 6+ months at Memorial
Skills & Competencies
-
In-depth knowledge of ICD-10, CPT, and HCC coding standards

Understanding of CMS regulations, LCD/NCD guidance, and billing workflows
-
Strong computer literacy and experience with EMR and HIM systems
-
Critical thinking and decision-making ability under minimal supervision
-
Effective communication with providers and cross-functional teams
-
Attention to detail and ability to work in a fast-paced setting
Physical Requirements
-
Mostly sedentary with occasional standing, walking, or light lifting
-
Heavy computer monitor use (80%) and repetitive hand/arm movement (60%)
-
Physical activities may include bending, kneeling, squatting, and reaching (up to 40%)
