Job Responsibilities

Care Coordination

  • Collaborate as part of a multi-disciplinary team to assist in coordinating medical and social services for patients and families.
  • Follow the plan of care, conducting face-to-face visits to assess progress and communicate updates to the care manager.
  • Assist in implementing interventions under the supervision of a licensed professional.
  • Communicate findings to the healthcare team and monitor patient needs to ensure efficient access to care and resources.
  • Provide self-management support, helping patients and families navigate chronic medical or psychosocial needs.

Scheduling

  • Accurately schedule patient appointments.
  • Answer inquiries regarding appointment preparations, clinic directions, and general hospital information.

Documentation

  • Maintain accurate and timely documentation related to the plan of care.

Communication

  • Update patients and families on progress and care plans, ensuring all information aligns with the plan of care.
  • Refer families to appropriate resources for support and additional information.
  • Collaborate with care team members to address complex healthcare needs.

Population Outreach

  • Assist in identifying at-risk populations and closing care gaps through targeted outreach.

Job Qualifications

  • High school diploma or equivalent.
  • 2+ years of experience in a related field.
  • Current Community Health Worker (CHW) certification in Ohio.
