Key Responsibilities of a Community Healthcare Assistant

As a Community Healthcare Assistant, you will:

  • Assist with Patient Care: Help deliver rehabilitation services and care in patients’ homes to ensure they are comfortable and well-supported.

  • Support Home Exercise Programs: Collaborate with the therapy team to assist patients with prescribed home exercise routines.

  • Provide Clerical Support: Assist the team with administrative tasks to ensure the smooth running of the service.

  • Report Patient Changes: Communicate any changes in a patient’s condition to the Operational Lead or Therapist in a timely manner.
