Job Description

We are looking for a dynamic and passionate Community Healthcare Assistant to join our dedicated team within the Horsham Primary Care Network (PCN) Community Team. This is a fantastic opportunity for individuals looking to start or develop their career in healthcare. As a Community Healthcare Assistant, you will be involved in delivering high-quality care, including tasks like insulin administration, wound care, and catheter care. While previous experience in community or healthcare settings is desirable, it’s not essential as comprehensive training will be provided to help you build the skills necessary for success in this rewarding role.

Key Responsibilities for Community Healthcare Assistant

Clinical Support & Patient Care

  • Undertake non-complex clinical tasks under supervision, including venepuncture, subcutaneous injections, and sample collection

  • Monitor patient conditions such as blood pressure, administer medications, and provide wound and catheter care

  • Empower patients by helping them regain health and independence in collaboration with the Multi-Professional Team

Working Across the Community as a Healthcare Assistant

Collaborative Team Support

  • Liaise with clients, family members, and other professionals to ensure coordinated and high-quality care

  • Accurately document patient records and escalate issues to senior team members as necessary

  • Show flexibility by working across various teams based on service needs
