Job Description
We are excited to offer a full-time opportunity for a Trainee/Assistant Practitioner to join our Integrated Stroke Rehabilitation Therapy Service. This role provides the chance to complete a Foundation Degree in Health and Social Care (Assistant Practitioner) through an apprenticeship route. Upon successful completion of the 2-year program, you will transition to a band 4 position. We are seeking a motivated and enthusiastic individual, preferably with experience in rehabilitation or an NVQ Level 3 in Health and Social Care. As a Trainee/Assistant Practitioner, you will support a multidisciplinary team of therapists in providing essential care for stroke patients across multiple settings, including hospitals, care homes, and patient homes.
Role and Responsibilities of a Trainee/Assistant Practitioner
Supporting Stroke Rehabilitation Therapies

Assist qualified therapists in delivering rehabilitation care to stroke patients, including tasks like assessing patient progress and implementing treatment plans

Gather assessment information, recommend equipment, and perform therapeutic activities with guidance from senior staff

Monitor and evaluate treatment programs, adjusting interventions under supervision as needed
Working Across Various Settings as a Trainee/Assistant Practitioner
Varied Work Environments and Community Care

Provide care in inpatient stroke units, care homes, and patients’ homes, including working as a lone worker in the community

Ensure patient care is tailored to individual needs and follows the guidelines set by senior therapists

Maintain clear communication with patients, families, and other healthcare professionals to ensure holistic care
