Job Summary

We have an exciting opportunity for an enthusiastic Health Care Assistant to join our friendly and supportive North Norwich Neighbourhood Community Nursing Team. Based at Norwich Community Hospital, you will support patients in their homes, residential settings, and clinic environments including our leg ulcer and catheter clinics.

You will work alongside a multi-professional team of nurses, nursing associates, assistant practitioners and fellow health care assistants, delivering person-centred care aimed at keeping patients out of hospital and improving quality of life.

Key Responsibilities

  • Deliver safe, delegated care under the supervision of registered practitioners

  • Support patients with a range of needs: diabetes, wound care, catheter care, palliative and end-of-life care

  • Work independently in community settings, prioritising and planning your own visits

  • Accurately complete patient records both on paper and using SystmOne

  • Take blood glucose readings, assist with wound care, and administer medications such as insulin (following training)

  • Communicate sensitively and effectively with patients and carers

  • Support health promotion including smoking cessation and falls prevention

  • Maintain cleanliness and infection control standards

  • Attend multidisciplinary meetings and training sessions

  • Respect confidentiality and uphold Trust values at all times

Requirements

  • Motivated, reliable team player with excellent communication skills

  • Ability to work independently within scope of practice

  • Clinical experience in a care setting is desirable

  • Willingness to complete required training and development

  • Understanding of consent, patient dignity, and diversity

  • Familiarity with community-based healthcare is an advantage
