Position Summary
As a Health Aide / CNA with Summit ElderCare, you’ll play a vital role in supporting participants both at our care center and in their homes. Under the supervision of an RN and as part of the interdisciplinary team, you will assist with activities of daily living (ADLs), help maintain a clean and safe environment, and contribute to our high standard of individualized care.
Key Responsibilities Include:

Assist with ADLs such as bathing, dressing, toileting, and transfers

Provide light housekeeping and meal support

Support recreational and therapeutic programs

Accurately document participant care using EMR systems

Participate in infection control and enhanced sanitation efforts per CDC guidelines

Collaborate with interdisciplinary team members to support care planning and delivery
Qualifications:

Completion of a Certified Nursing Assistant (CNA) or Home Health Aide training program is preferred (relevant experience may substitute)

Certification as a CNA or HHA is strongly desired

Minimum one year of experience with frail or elderly individuals, preferably in a PACE setting (those with less experience may qualify with completion of our internal training program)

Must complete annual competency assessments and training

Reliable transportation is required for community-based assignments
