Job Description

Position Purpose:

This position encompasses professional roles in the pharmacy department, involving supervisory, clinical, or administrative duties. The role offers varying levels of autonomy for independent decision-making and judgment. Health System Pharmacists are expected to comply with all federal, state, and local laws, as well as other regulatory requirements related to pharmacy and institutional practice.

Key Responsibilities:

  • Accurately fill prescriptions and patient orders, resolving any discrepancies in patient therapy and following up on issues as needed.

  • Utilize clinical data to evaluate physician orders and ensure appropriate therapy is administered.

  • Provide drug information and counseling to patients, documenting all interventions, adverse drug reactions (ADRs), and medication errors.

  • Review clinical information to assess the appropriateness of therapy and make necessary adjustments.

  • Ensure effective communication with hospital staff regarding patient therapy and updates.

  • Maintain accurate patient records, reviewing orders via the pharmacy computer system.

  • Prepare and maintain necessary files and reports.

  • Participate in pharmacy staff in-services and hospital-wide meetings.

  • Communicate important information regarding patient therapy to colleagues and healthcare professionals during handoffs.

  • Collect data and prepare reports as needed.

  • Be proficient in pharmacy and related computer systems.

Minimum Qualifications:

  • A valid New York State pharmacy license.
