Job Description

Position Purpose:

This role involves professional staff members with supervisory, clinical, or administrative responsibilities within a pharmacy department, offering varying levels of independence and decision-making authority. Health System Pharmacists are expected to comply with all federal, state, and local laws, as well as other regulatory requirements pertaining to pharmacy and institutional practices.

Duties & Responsibilities:

  • Accurately fills prescriptions and patient orders, addresses discrepancies in patient therapy, and follows up on issues.

  • Evaluates physician orders using clinical data to ensure the appropriateness of therapy.

  • Provides drug information and counseling to patients, and documents these interventions.

  • Records adverse drug reactions (ADRs) and medication errors, and documents these in patient records.

  • Reviews clinical information to assess the suitability of patient therapy.

  • Resolves therapy discrepancies and follows up on issues in a timely manner.

  • Provides essential drug information to hospital staff and healthcare team members.

  • Conducts effective patient counseling on medication use.

  • Records and reviews patient orders through the pharmacy computer system.

  • Prepares and maintains patient files and reports.

  • Participates in pharmacy staff in-services and hospital-wide meetings.

  • Communicates effectively with colleagues and healthcare professionals to ensure the accurate exchange of information related to patient therapy.

  • Collects data and prepares relevant reports as required.

  • Proficient in using pharmacy computer systems and related technology.

Minimum Qualifications:

  • Valid NY State license to practice pharmacy.
