Job Description

Position Purpose:

This role involves professional staff members with supervisory, clinical, or administrative responsibilities within a pharmacy department, offering varying levels of independence and decision-making authority. Health System Pharmacists are expected to comply with all federal, state, and local laws, as well as other regulatory requirements pertaining to pharmacy and institutional practices.

Duties & Responsibilities:

Accurately fills prescriptions and patient orders, addresses discrepancies in patient therapy, and follows up on issues.
Evaluates physician orders using clinical data to ensure the appropriateness of therapy.
Provides drug information and counseling to patients, and documents these interventions.
Records adverse drug reactions (ADRs) and medication errors, and documents these in patient records.
Reviews clinical information to assess the suitability of patient therapy.
Resolves therapy discrepancies and follows up on issues in a timely manner.
Provides essential drug information to hospital staff and healthcare team members.
Conducts effective patient counseling on medication use.
Records and reviews patient orders through the pharmacy computer system.
Prepares and maintains patient files and reports.
Participates in pharmacy staff in-services and hospital-wide meetings.
Communicates effectively with colleagues and healthcare professionals to ensure the accurate exchange of information related to patient therapy.
Collects data and prepares relevant reports as required.
Proficient in using pharmacy computer systems and related technology.

Minimum Qualifications:

Valid NY State license to practice pharmacy.
