Job Summary
Surrey Downs Health and Care is looking to recruit a committed Band 3 Healthcare Assistant to join our District Nursing Team within the Cobham and Oxshott Primary Care Network.
You will support the provision of person-centred nursing care in patients’ homes and community settings. This role includes clinical duties such as wound care and phlebotomy, along with key administrative responsibilities. A valid UK driving licence and access to a vehicle are essential, as this is a community-based role requiring travel between patient visits.
Main Duties and Responsibilities
Clinical Duties

Support with clinical interventions under supervision (e.g., wound care, Doppler testing, phlebotomy, bladder screening).

Perform diagnostic tests for chronic disease monitoring as requested by District Nurses, Community Matrons, or GPs.

Conduct regular home visits and support ongoing testing programmes in residential settings.
Health Promotion

Contribute to holistic care assessments and implementation plans.

Support, educate, and inform patients and carers where appropriate.

Promote health and wellbeing in the community.
Administration and Record-Keeping

Maintain accurate, signed, and up-to-date care records in accordance with policy.

Manage incoming communications and documentation supplies.

Maintain data records such as mileage logs, rotas, and leave on staff systems.
Team Collaboration

Work collaboratively with nurses, healthcare professionals, and wider teams.

Participate in regular team meetings and handovers.

Assist with clerical tasks to support service efficiency.

Person Specification
Qualifications

Essential: NVQ Level 3 in Health and Social Care or equivalent

Desirable: Evidence of recent professional development or healthcare training
Experience

Essential: Previous experience in health or social care settings

Desirable: Clerical/administrative/IT experience
