Summary
The Medical Coder is responsible for accurately coding and abstracting hospital medical records (Inpatient, Observation, Outpatient Surgery, Invasive Outpatients, and Emergency Department) using ICD-10-CM, ICD-10-PCS, CPT-4, and encoding software. The role requires adherence to federal/state guidelines, DRG validation, and collaboration with clinical documentation improvement (CDI) staff.
Key Responsibilities

Review and code medical records for Inpatient, Observation, Outpatient Surgery, Invasive Outpatients, and Emergency Department

Sequence principal diagnoses and procedures according to documentation and UHDDS definitions

Utilize ICD-10-CM, ICD-10-PCS, CPT-4, and encoder tools accurately

Ensure compliance with AHA, AHIMA, UHDDS, and federal/state guidelines

Validate DRG assignments using current coding knowledge

Abstract required data into computer systems and follow downtime procedures as needed

Participate in departmental performance improvement and maintain competency standards

Collaborate with clinical staff to clarify documentation
Qualifications & Skills

Minimum 2 years inpatient records coding experience (or equivalent)

Knowledge of Anatomy & Physiology and medical terminology

Proficiency in Microsoft Windows environment

Attention to detail with high-level accuracy

Strong decision-making, communication, and teamwork skills

Previous experience with electronic health records preferred
Certifications

AHIMA Certification preferred:

Certified Coding Specialist (CCS)
