Position Overview
Concho Hearts Hospice is seeking a compassionate and skilled Registered Nurse (RN) to provide high-quality, individualized nursing care to patients of all ages in their homes. This position involves close coordination with an interdisciplinary healthcare team, the patient, their family, and referring agencies to ensure continuity and quality of care. The RN will also play a key role in educating caregivers, offering emotional support, and promoting patient comfort and dignity, especially during end-of-life care.
Key Responsibilities

Develop, implement, and revise individualized patient care plans

Deliver direct nursing care based on best practices and patient needs

Educate and support family members and caregivers, especially regarding end-of-life processes

Maintain accurate, up-to-date medical records and documentation in accordance with standards

Coordinate care across disciplines to ensure holistic and seamless treatment

Respond to PRN calls and urgent needs as required
Qualifications

Education: Graduate of an accredited diploma, associate, or bachelor’s program in nursing

Licensure:
-
Active RN license (in good standing)
-
Eligible under the Nurse Licensure Compact (NLC)

Current CPR certification
-
Valid driver’s license and auto liability insurance

Experience:
-
Minimum two years of clinical nursing experience preferred

Prior experience in home health, hospice, palliative care, or end-of-life care is a plus

Demonstrated nursing proficiency aligned with professional standards

Excellent interpersonal and communication skills

Ability to assess and respond to both physical and emotional patient needs

Proficient in documentation and record-keeping

Ability to handle high-pressure situations and make timely decisions

Work Environment & Conditions

Care is provided in patients’ homes under varying conditions

Exposure to infectious diseases and bodily fluids may occur

Must be able to work flexible hours and respond to emergency PRN calls

Occasional travel required within the local service area

May be exposed to inclement weather and other environmental challenges
Physical & Mental Demands

Must be able to stand and walk for extended periods

Ability to lift and assist patients, up to 50 pounds

Requires emotional resilience and the ability to remain calm under stress

Must be capable of making rapid decisions in emergency scenarios
