Job Summary
We are seeking a Pharmacist to join our team at Nantucket Cottage Hospital. This role is responsible for optimizing drug therapy through identification, resolution, and prevention of drug-related problems to improve patient outcomes. The pharmacist will provide direct patient care in compliance with state and federal regulations and may prescribe medications under Collaborative Drug Therapy Management (CDTM) agreements.
Key Responsibilities

Direct and coordinate pharmacy services to ensure safe, effective, and compliant medication use.

Verify the accuracy of all pharmacy work completed by support staff during assigned shifts.

Ensure compliance with formulary management, target drug programs, and adverse drug event monitoring.

Document and report medication errors, clinical interventions, and adverse drug events.

Support departmental quality improvement initiatives and participate in hospital committees.

Provide drug information, education, and training to patients, medical staff, and allied health professionals.

Maintain compliance with Board of Pharmacy regulations and organizational policies.

Participate in staff education, training, and mentorship activities.

Complete continuing education requirements (20 CE credits annually).
Qualifications
Education
-
Doctor of Pharmacy (PharmD) degree required.
Licenses & Credentials

Massachusetts Pharmacist license required.

Massachusetts Controlled Substances Registration (MCSR) preferred.
Experience
-
2–3 years of pharmacist experience preferred (hospital or clinical setting).
Knowledge, Skills, and Abilities
-
Strong knowledge of IV admixtures, drug distribution, ambulatory care, and medication safety.

Ability to assess and provide age-specific care across neonatal, adolescent, adult, and geriatric populations.

Familiarity with prescription processing, drug identification, generics, manufacturing, and storage practices.

Strong attention to detail, communication skills, and ability to work independently.
