The Clinical Pharmacist at TriHealth plays an essential role in optimizing patient care through expert pharmaceutical guidance and collaboration across clinical teams. This position combines clinical expertise with supervisory responsibilities, contributing to the safe and effective use of medications and supporting both patient outcomes and departmental goals.
Key Responsibilities

Patient-Centered Care: Conduct thorough patient assessments and provide evidence-based medication recommendations to enhance therapeutic outcomes and reduce adverse effects.

Clinical Collaboration: Maintain open, professional communication with physicians, nurses, pharmacists, technicians, and leadership to ensure coordinated, high-quality care.

Patient Education: Provide clear, timely counseling on medication therapies to patients and their caregivers.

Regulatory & Safety Compliance: Ensure adherence to the “seven rights” of medication administration (right patient, drug, dose, route, time, information, and documentation). Report and support mitigation of medication errors.

Medication Management: Oversee medication distribution including order verification, technician supervision, and timely delivery. Monitor and adjust drug inventory to prevent shortages and minimize waste.

Process Improvement: Identify and implement pharmacy-related process enhancements; support the development of protocols that improve medication safety and cost-effectiveness.

Leadership & Mentorship: Serve as a preceptor for pharmacy students and residents. Assist with training, orientation, and departmental projects.

Committee Participation: Contribute to pharmacy initiatives and quality improvement projects in collaboration with Pharmacy Leadership.
Qualifications
Education & Licensure:

Bachelor’s degree in Pharmacy required (Doctor of Pharmacy preferred).

Active Pharmacist license in the state of Ohio.

Knowledge of Ohio Board of Pharmacy regulations.
Experience:

1–2 years of relevant pharmacy practice experience preferred.
Skills:

Proficiency in dosage calculations and prescription formulas.

Strong analytical, interpersonal, and problem-solving skills.

Ability to work effectively within multidisciplinary teams.
