Keys Responsibilities

This role focuses on Transitions of Care management, specifically medication management for patients discharged from acute care hospitals. The goal is to ensure medication access, accurate discharge medication reconciliation, and provide post-discharge follow-up care. The position involves delivering comprehensive pharmaceutical care, including patient-centered clinical services, medication information, education, preparation, and distribution to ensure safe, effective, and cost-efficient medication therapy.

The pharmacist will use sound judgment and clinical expertise to provide input into the patient’s care plan based on their diagnosis, ensuring timely interventions. By building trust and cooperation with patients, their families, and other healthcare providers, the pharmacist will monitor and manage patient care effectively. Adherence to local, state, and federal regulations, as well as company policies and procedures, is essential to ensure privacy, safety, and optimal patient care. This role may also include performing specific tasks or orienting new staff within the department.

Education

  • Doctorate: Graduation from an accredited school of Pharmacy.

Certification & Licensure

  • PHARMD-Licensed Pharmacist or

  • PHARMR-Registered Pharmacist

  • ACLS-Advanced Cardiac Life Support (required for those responding to codes)

Typical Experience

  • 1 year of recent relevant experience.

  • Transitions of Care experience preferred.

Skills and Knowledge

  • In-depth knowledge of medical terminology, generic and trade pharmaceutical names, pharmaceutical calculations, and relevant laws and regulations.

  • Understanding of various Pharmacy practice settings, including narcotic delivery procedures.

  • Ability to interpret pharmaceutical and medical terms, abbreviations, and symbols commonly used in prescribing, dispensing, and record-keeping.

  • Familiarity with legal requirements and accreditation standards, such as NABP, TJC, Title XXII, DHS, DEA, FDA, and USP.

  • Strong written and verbal communication skills to convey complex information clearly and professionally to diverse audiences, including non-medical individuals.

  • Well-developed time management and organizational skills, with the ability to prioritize tasks and meet deadlines within established procedures.

  • General knowledge of computer applications, including Microsoft Office Suite (Word, Excel, Outlook), EHR systems, and EPIC.

  • Ability to work independently and as part of a team, managing multiple tasks in a dynamic environment.

  • Problem-solving skills to identify and resolve issues using established solutions.

  • Commitment to maintaining patient privacy and confidentiality (PHI).
