Responsibilities:

  • Ensure quality care by following physician-approved standards.

  • Provide in-home care to patients.

  • Document clinical notes accurately for the clinical record.

  • Administer treatments, medications, and perform health assessments as ordered.

  • Communicate with case managers and physicians as needed.

  • Complete both electronic and paper documentation in compliance with state and federal guidelines.

  • Participate in required continuing education.

Qualifications:

  • Current, active LPN license.

  • CPR certification.

  • Knowledge of OSHA, FDA, and HIPAA regulations.

  • Ability to lift up to 50 pounds, pass a fit-to-work physical, and be free of communicable diseases.
