We are seeking a dedicated Licensed Practical Nurse (LPN) to support our clinical team by delivering high-quality patient care and ensuring smooth day-to-day clinic operations. This role involves a mix of clinical responsibilities, front office support, and patient engagement, helping promote health and wellness within our community.
Your Impact
As an LPN, you will play a vital role in patient care delivery by:
Patient Care Duties

Record vital signs and patient information in the electronic medical record (EMR).

Administer immunizations, oral medications, and injections per provider orders across all age groups.

Assist with diagnostic tests, procedures, and point-of-care testing.

Observe and document patient behavior and treatment responses.

Perform phlebotomy and process lab specimens.
-
Educate patients on self-care and chronic condition management under provider guidance.

Coordinate follow-up care, referrals, and health screenings.

Conduct evidence-based phone triage using clinical judgment and protocols.
Clinic Operations

Manage patient flow and clinic efficiency.

Prepare exam/procedure rooms with proper aseptic techniques and equipment.

Handle front-desk tasks including scheduling, phones, and patient registration as needed.

Maintain inventory and order clinical supplies.

Support clinic initiatives and off-site events when required.
Compliance & Documentation

Perform quality assurance checks and maintain accurate records.

Follow all infection control and HIPAA protocols.

Ensure documentation meets audit standards and regulatory compliance (OSHA, CLIA).
Team Support

Participate in team meetings, daily huddles, and collaborative care planning.

Engage in training, competency assessments, and continuous learning.

Support performance goals for patient care and service delivery.

Flexibility to work varying shifts and perform additional duties as needed.
What You’ll Bring
Education & Certification
-
High school diploma or GED required.
-
Graduate of an accredited LPN/LVN program.
-
Current state license as a Licensed Practical Nurse.
-
CPR certification (BLS) required.
Experience
-
Minimum of 2 years’ nursing experience preferred (primary or urgent care settings ideal).

Proficiency in using EMR systems.

Comfortable administering immunizations and assisting with clinical procedures.

Health & Safety Requirements

Proof of Hepatitis B vaccination or immunity.

Negative TB test within 90 days of start date.

Current COVID-19 and annual flu vaccination per company policy.
