Position: Home Health Licensed Practical Nurse (LPN)
Location: CenterWell Home Health
Schedule: Full-Time | 40 Hours Weekly
Pay Range: $44,600 – $61,400 annually or $28 – $40 per visit/unit
(Compensation is based on experience, skills, location, and certifications.)
Your Role as a Home Health LPN
As a Home Health LPN, you’ll work under the supervision of a Registered Nurse to deliver high-quality nursing care in alignment with your state’s Nurse Practice Act, healthcare regulations, and CenterWell’s standards and policies. This role offers an opportunity to make a direct impact on patients’ lives in the comfort of their homes.
Key Responsibilities:

Provide skilled nursing care as outlined in the patient’s plan of care.

Assist in holistic care planning and deliver therapeutic interventions to support rehabilitation and preventive care.

Monitor and document patient progress, responding to changes in condition and updating care plans accordingly.

Collaborate with physicians, RNs, and other care team members to ensure comprehensive patient care.

Educate patients and caregivers on health maintenance and treatments.

Maintain timely and accurate documentation of services rendered.

Ensure proper use of supplies and medical equipment for safe, effective care delivery.

Maintain patient confidentiality and adhere to professional standards of conduct.
Required Qualifications:

Graduate of an accredited LPN or LVN program.

Active LPN/LVN license in the practicing state.

Valid driver’s license, reliable transportation, and auto insurance.

Current CPR certification.
-
At least 2 years of LPN/LVN experience in a clinical setting (home health or hospice experience preferred).
