Your Role as a Home Health LPN

Deliver professional nursing care under the supervision of a Registered Nurse (RN), in accordance with the Nurse Practice Act, licensure requirements, healthcare regulations, and CenterWell Home Health policies.

Provide skilled nursing services that address patients’ medical conditions, rehabilitative goals, and preventive health needs using a patient-centered, holistic approach.

Apply clinical knowledge to provide therapeutic interventions, patient education, and goal-driven care tailored to each individual’s plan of care.

Communicate effectively with the RN Supervisor to recommend updates to the Plan of Treatment and relay verbal physician orders when allowed by state law and regulation.

Implement nursing care plans using a structured, outcome-based approach.

Coordinate with patients, families, physicians, and the care team to ensure timely communication regarding patient status, needs, and changes in condition.

Support care coordination and participate in discharge planning when appropriate.

Maintain strict confidentiality and share sensitive information only when it aligns with patient rights and best interests.

Ensure timely access to necessary supplies, equipment, and materials required to carry out patient care safely and efficiently.

Accurately document visits, treatments, and patient interactions on the same day services are delivered, in compliance with organizational standards.
Qualifications & Experience
Use Your Skills to Make a Lasting Impact

Graduate of an accredited Practical/Vocational Nursing Program

Active and unrestricted LPN license in the state of practice

Current CPR certification

Valid driver’s license, automobile insurance, and reliable transportation

Minimum of two years’ clinical experience as an LPN—home health or hospice experience preferred
Work Schedule

Full-Time: 40 hours per week
Compensation
-
Per Visit/Unit Pay: $28.00 – $40.00

Annual Base Salary: $44,600 – $61,400
