About the Job
Medi Home Health Agency, a division of Medical Services of America Inc., is currently seeking a skilled Licensed Practical Nurse (LPN) to provide care for our home health patients in Scottdale (Westmoreland), PA.
As an integral part of a multidisciplinary healthcare team, the LPN will perform designated nursing duties under the supervision of a Registered Nurse.
Key Responsibilities:

Deliver skilled nursing care to patients in accordance with the care plans established by the Registered Nurse and physician, following agency protocols.

Adhere strictly to all agency policies and procedures.

Support continuity of patient care by collaborating and communicating effectively with other healthcare professionals.

Prepare detailed clinical progress notes and implement care plans as directed by the Registered Nurse, consistent with agency standards.

Participate in on-call rotations as required.
Qualifications:

Completion of a certified Licensed Practical Nursing program.

Active LPN license in the state of Pennsylvania.

Minimum of one year of nursing experience.

Prior home health care experience is strongly preferred.
