Position Overview:
The Licensed Practical Nurse (LPN) is responsible for delivering nursing care and performing medical procedures within the scope defined by their nursing license. The LPN supports the integration of physical and behavioral health services within the program and reports directly to the Registered Nurse (RN) or Nursing Leadership.
Key Responsibilities:

Administer prescribed and routine medical treatments in accordance with physician orders, ensuring strict adherence to internal policies and documentation standards.

Assist with scheduling client appointments and gathering necessary medical records prior to visits.

Serve as a liaison between external healthcare providers, educational institutions, community organizations, and the facility concerning clients’ medical needs.

Provide general nursing care within the LPN scope, including:
-
Taking and recording vital signs.
-
Administering prescribed medications, documenting dosage and timing accurately in client charts.

Monitoring clients for adverse medication or treatment reactions and reporting concerns to supervisors.

Performing phlebotomy and preparing specimens for laboratory testing.
-
Managing inventory and ordering of medical supplies as needed.

Respond promptly to medical emergencies, delivering triage and stabilizing clients within the LPN scope of practice, followed by appropriate medical follow-up.

Complete all required documentation and charting per internal guidelines and regulatory requirements.

Perform additional duties within the LPN New York State license scope as directed by the RN or Nursing Leadership.

Education and Training Requirements:

Active Licensed Practical Nurse (LPN) license in the state of New York.

Current CPR certification is mandatory.

Proficient in both oral and written communication, with strong collaboration skills.

Ability to walk, stand, sit, and bend as needed to provide effective client care in various settings.

Capable of responding quickly to emergencies, including performing CPR and utilizing emergency protocols.

Skilled in safely lifting, transferring, and repositioning clients to ensure safety and comfort.

Stamina to maintain nursing duties throughout the work shift, including extended periods of standing and walking.

Ability to navigate diverse environments, including confined spaces, while maintaining patient care and safety.
