Key Responsibilities

The LPN’s duties include, but are not limited to:

  • Providing respectful, non-judgmental care in accordance with the established Patient Care Plan.

  • Applying knowledge of age- and population-specific factors that influence Veteran health needs.

  • Supporting admission and discharge procedures as directed by the care team.

  • Gathering patient data and reporting significant observations and changes in condition.

  • Recognizing and responding appropriately to urgent or emergent patient care needs.

  • Communicating patient status and care responses to the supervising RN or provider.

  • Organizing and prioritizing daily care responsibilities.

  • Delivering age-appropriate care that meets physical, emotional, cognitive, and developmental needs.

  • Accurately documenting clinical observations and care using the electronic medical record.

  • Providing patient/family education on disease processes, medications, and prescribed treatments as directed.

  • Preparing, administering, and documenting medications using the Bar Code Medication Administration (BCMA) system.

  • Monitoring and reporting adverse reactions to medications to the RN or provider.


Work Schedule

  • Hours: Monday–Friday, 8:00 AM–4:30 PM (PST)

  • Telework: Not authorized

Conditions of Employment

  • Open to all qualified candidates, including U.S. citizens. Non-citizens may be considered if no qualified citizens are available.

  • Pre-employment drug screening (urinalysis) is required for testing-designated positions.

  • Proficiency in spoken and written English is required.

  • Completion of online onboarding and submission of acceptable identification documentation is required.

  • Starting May 7, 2025, only REAL ID-compliant licenses or IDs will be accepted for employment processing.

  • Participation in the seasonal flu vaccination program is mandatory for VA healthcare personnel.


Basic Qualifications

  • Citizenship: Must be a U.S. citizen (non-citizens may be appointed per VA policy when necessary).

  • Language Proficiency: Must be fluent in spoken and written English.

  • Education: Must be a graduate of an approved school of practical or vocational nursing. Military training and certain grandfathering provisions may qualify.

  • Licensure: Must hold an active, full, current, and unrestricted license as an LPN/LVN in a U.S. state, territory, or the District of Columbia.


Grade-Level Qualifications

  • GS-3: No additional experience required beyond basic qualifications.

  • GS-4:

    • At least six months of qualifying LPN experience, OR

    • Graduation from an approved program and one year of healthcare experience in a supervised setting.

  • GS-5:

    • One year of LPN experience at the GS-4 level, with demonstrated competencies including:

      • Providing nursing care for patients with a range of physical/behavioral conditions.

      • Educating patients/families about conditions, treatments, and medications.

      • Administering and documenting medications.

      • Responding to emergencies and supporting resuscitation procedures.

      • Identifying and addressing equipment/safety issues.

  • GS-6 (Full Performance Level):

    • One year of experience at the GS-5 level, with proficiency in:

      • Providing care to diverse patient populations using clinical judgment.

      • Administering medications and treatments, observing and documenting patient responses.

      • Recognizing and responding to emergency situations.

      • Supporting diagnostic procedures and specialized care practices.

      • Promoting patient education and establishing therapeutic relationships.

      • Sharing knowledge with peers to enhance nursing practice.


Preferred Experience

  • One or more years of experience providing direct patient care in a primary care setting.


Physical Requirements

This role demands physical and mental stamina. The LPN must possess visual acuity, hearing, clear communication, and manual dexterity. Duties may involve extended periods of walking, standing, bending, and lifting, including assisting in patient transfers. Personal protective equipment must be worn as required. Exposure to infectious materials, combative patients, and high-stress environments may occur. Annual health screenings, including TB testing, are mandatory.
