  • Responsibilities

    Clinical, Technical & Patient Care

    • Collaborate with the program psychiatrist to assess the medical and psychiatric needs of patients.
    • Serve as a liaison between patients and their primary care providers to ensure comprehensive care.
    • Conduct health evaluations and follow-ups for referred patients, including medication management under the supervision of a physician.
    • Assess and address urgent health concerns, coordinate referrals, and review laboratory reports.
    • Provide education, treatment, rehabilitation, and support services to patients, including community-based care when necessary.
    • Work closely with the program nurse and multidisciplinary team to develop and implement integrated treatment plans.
    • Actively participate in team meetings and case rounds to support collaborative decision-making.
    • Maintain accurate and timely documentation in patient medical records.

    Organizational & Compliance Responsibilities

    • Contribute to departmental performance improvement initiatives.
    • Ensure patient and employee confidentiality when managing sensitive information.
    • Adhere to health system compliance policies and regulatory guidelines.

    Education & Professional Development

    • Support the professional growth and training of fellow team members.
    • Meet regulatory, licensure, and health assessment requirements as mandated.
    • Identify personal learning needs and opportunities for skill enhancement.

    • Demonstrate patience, tact, and diplomacy in all interactions with hospital staff and community partners.

    Qualifications

    • BSN or LPN required.
    • Experience working with individuals with mental illness and co-occurring substance use disorders is essential.
    • New York State Registered Nurse (RN) license preferred; Licensed Practical Nurses (LPNs) will also be considered.
