Position Summary

LifeMoves | Branham Lane, located in South San Jose, provides comprehensive support services to single adults and multi-generational families experiencing homelessness. The program offers on-site services and connections to external resources, including housing assistance, basic needs support, financial literacy workshops, resume development, nutrition education, and conflict resolution. The overarching goal is to help clients achieve self-sufficiency and transition into stable, permanent housing.

LifeMoves is currently seeking a compassionate and experienced Licensed Vocational Nurse (LVN) to join our team in delivering high-quality, shelter-based care for homeless individuals in Santa Clara County. This position is ideal for a flexible, solutions-oriented professional who thrives in a fast-paced environment. The ideal candidate will bring expertise in issues impacting marginalized populations, including physical and mental health challenges and co-occurring disorders.

Key Responsibilities

  • Conduct comprehensive assessments and develop individualized case plans for shelter residents

  • Implement, revise, and monitor progress on client care plans with a focus on housing and wellness goals

  • Provide ongoing case management and supportive services as outlined in care plans

  • Coordinate with medical professionals, social workers, and family members as appropriate to ensure continuity of care

  • Assist clients in accessing benefits and community resources aligned with treatment goals

  • Make independent decisions regarding client care when supervisory support is unavailable

  • Facilitate client linkages to external systems for crisis intervention, advocacy, and long-term housing or employment support

  • Maintain accurate and timely documentation in compliance with regulatory and grant requirements

  • Uphold high standards of ethical conduct, including confidentiality and professional boundaries

Qualifications

  • Graduate of an accredited nursing program

  • Current and valid LVN license in the State of California

  • Strong interpersonal and communication skills, with the ability to collaborate effectively across diverse stakeholders

  • Knowledge of the challenges facing individuals experiencing homelessness and marginalized populations

  • Experience with individuals who have mental health conditions or co-occurring disorders is strongly preferred

  • Bilingual in Spanish is a plus
