Position Summary
Telecare is seeking a compassionate and skilled Licensed Vocational Nurse (LVN) or Licensed Psychiatric Technician (LPT) to join our direct care team. In this role, you’ll support individuals with complex behavioral health needs by administering prescribed medical treatments and medications, ensuring safe, effective, and person-centered care in alignment with clinical standards and organizational protocols. This position reports directly to the Program Administrator.
Key Responsibilities

Embody and promote Telecare’s mission, values, and purpose in all professional interactions

Deliver direct care to members in both one-on-one and group environments, using structured teaching methods and natural settings

Build and maintain therapeutic relationships with clients

Assist with admissions, discharges, and overall nursing care

Gather and document client data to inform care planning

Respond to behavioral crises with effective intervention strategies
-
Accurately transcribe physician orders and administer medications and treatments
-
Provide timely shift reports and ensure proper medication documentation and hand-offs
-
Monitor and communicate changes in client condition to clinical leadership
-
Track member progress toward individualized recovery goals
-
Participate in treatment team meetings and care planning discussions

Support medication counts and proper documentation across shifts

Participate in crisis management and physical de-escalation interventions as needed

Assist the LVN/LPT Direct Care Lead in skill-building assessments, behavior plans, and material preparation

Fill in for the LVN/LPT Direct Care Lead during absences
-
Complete monthly supervision meetings with qualified supervisors; 5% of activities must be supervised monthly by a BCaBA or BACA-certified staff
Additional duties may be assigned based on program needs.
Qualifications
Required:
-
High school diploma or GED
-
Graduate of an accredited LVN or LPT program with active state licensure

Valid CPR/BLS certification
-
At least 6 months of direct care experience with individuals with developmental disabilities

Must begin Registered Behavior Technician (RBT) training within 6 months and earn certification within 12 months (unless already credentialed)

Must complete Direct Support Professional (DSP) competency training within 1 year unless proof of prior completion is provided

Minimum age: 18

Must have CPR, CPI, and First Aid certification (hands-on) prior to direct care duties

Must pass background checks, TB testing, physical exams, and meet all licensing and driving requirements (if applicable)
Preferred:
-
Experience working in behavioral health or similar care environments

Knowledge of psychiatric conditions and treatments, including psychotropic medications
-
Bilingual skills are a plus

Proficient in administering medications, including injectables

Skilled in crisis de-escalation techniques and behavioral support

Ability to document care and monitor progress toward recovery goals
Physical Requirements

Must be able to frequently walk, stand, bend, and lift up to 25 lbs

Requires manual dexterity and the ability to respond to physical emergencies

Exposure to bodily fluids, medical waste, and hazardous materials is possible
