Care Navigator Responsibilities
As a Care Navigator, you’ll play a key role in ensuring patients feel supported and connected:

Conduct virtual intake interviews to collect medical history and mental health concerns.

Schedule and coordinate telehealth sessions with licensed providers.

Update and manage patient information in electronic health records (EHR).

Handle communication via calls, emails, and patient portals.

Provide follow-up support to ensure treatment adherence.

Assist with billing, insurance claims, and administrative documentation.

Distribute educational resources and mental health materials.

Triage patient inquiries and direct crisis cases to the right resources.

Qualifications
