Key Responsibilities for a Medical Records Coder II
Accurate Medical Coding & Documentation

Review complex medical records and accurately code primary and secondary diagnoses and procedures using ICD-10-CM and CPT-4 conventions.

Ensure proper sequencing of diagnoses and procedures following coding guidelines.

Verify DRG/APC assignment accuracy and abstract data to optimize reimbursement for hospital and professional charges.

Assist with special projects, audits, and quality improvement initiatives as needed.
Team Coordination & Training

Coordinate and review coding work performed by designated employees to maintain quality and productivity.

Assist in developing, presenting, and supporting continuing education programs for coding staff and physicians.

Consult with physicians, nursing, and ancillary personnel to ensure complete and accurate documentation.

Maintain coding standards while mentoring and providing technical guidance to colleagues.
Knowledge & Compliance
Stay Current in Coding Practices

Maintain thorough understanding of anatomy, physiology, medical terminology, disease processes, and surgical techniques.

Keep up to date with coding guidelines, federal and state regulations, and accreditation standards (JCAHO, HCFA, MRNC, etc.).

Apply best practices to coding inpatient and outpatient diagnoses and procedures.

Participate in continuing education to ensure coding accuracy and professional growth.
Qualifications & Certifications

High school diploma required; additional education preferred.

Must hold one of the following active certifications:

RHIA (Registered Health Information Administrator)

RHIT (Registered Health Information Technician)

CCS (Certified Coding Specialist)

CPC (Certified Professional Coder)

HCS-D (Homecare Coding Specialist-Diagnosis)

Required experience:

RHIA or RHIT: no prior experience required

CCS: 1 year of coding experience

CPC or HCS-D: 2 years of coding experience
