Registered Nurse (RN) Case Manager – Hybrid Opportunity (No Prior Case Management Experience Required)

We’re seeking a dedicated and empathetic Registered Nurse (RN) Case Manager to support individuals with complex medical and social needs. This hybrid role offers a balance of remote work and local field visits, providing a meaningful alternative to traditional bedside care.


Key Responsibilities:

  • Perform comprehensive assessments to evaluate clinical, psychosocial, and environmental needs.

  • Develop and revise individualized care plans in collaboration with interdisciplinary teams and according to physician directives.

  • Complete MassHealth MDS assessments in accordance with program requirements.

  • Educate and support members and families regarding healthcare needs and available resources.

  • Coordinate with Commonwealth Care Alliance (CCA), Element Care staff, and external community providers to ensure continuity of care.

  • Manage a caseload of approximately 70–80 members once fully trained.

  • Engage in team knowledge-sharing activities and contribute to ongoing quality improvement initiatives.

  • Perform additional duties as required.


Training & Onboarding:

  • Participate in a 2–4 week structured training program alongside experienced RN Case Managers.

  • Receive ongoing mentorship and support as you transition into managing an independent caseload.

Required Qualifications:

  • Current Massachusetts RN license in good standing.

  • At least 2 years of nursing experience.

  • Willingness and ability to travel locally for in-home visits.

  • Strong clinical assessment, communication, and organizational skills.


Preferred Qualifications:

  • Experience in Home Health, VNA, PACE programs, or conducting MDS assessments.

  • Bachelor of Science in Nursing (BSN).

  • Bilingual skills are a plus.

  • Certified Case Manager (CCM) certification is preferred but not required.
