Registered Nurse (RN) Case Manager – Hybrid Opportunity (No Prior Case Management Experience Required)
We’re seeking a dedicated and empathetic Registered Nurse (RN) Case Manager to support individuals with complex medical and social needs. This hybrid role offers a balance of remote work and local field visits, providing a meaningful alternative to traditional bedside care.
Key Responsibilities:

Perform comprehensive assessments to evaluate clinical, psychosocial, and environmental needs.

Develop and revise individualized care plans in collaboration with interdisciplinary teams and according to physician directives.

Complete MassHealth MDS assessments in accordance with program requirements.

Educate and support members and families regarding healthcare needs and available resources.

Coordinate with Commonwealth Care Alliance (CCA), Element Care staff, and external community providers to ensure continuity of care.

Manage a caseload of approximately 70–80 members once fully trained.

Engage in team knowledge-sharing activities and contribute to ongoing quality improvement initiatives.

Perform additional duties as required.
Training & Onboarding:

Participate in a 2–4 week structured training program alongside experienced RN Case Managers.

Receive ongoing mentorship and support as you transition into managing an independent caseload.

Required Qualifications:

Current Massachusetts RN license in good standing.

At least 2 years of nursing experience.

Willingness and ability to travel locally for in-home visits.

Strong clinical assessment, communication, and organizational skills.
Preferred Qualifications:

Experience in Home Health, VNA, PACE programs, or conducting MDS assessments.

Bachelor of Science in Nursing (BSN).

Bilingual skills are a plus.

Certified Case Manager (CCM) certification is preferred but not required.
