Job Overview
We are currently seeking compassionate and skilled Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) to join our dedicated healthcare team. In this role, you will be responsible for delivering high-quality patient care, performing thorough assessments, and developing personalized care plans. The ideal candidate is patient-focused, detail-oriented, and able to collaborate effectively with interdisciplinary healthcare teams to ensure the best outcomes for our patients.
Key Responsibilities

Conduct comprehensive assessments to evaluate patient health status and identify needs

Develop and implement individualized care plans based on assessment findings

Administer IV infusions and medications as prescribed

Perform phlebotomy procedures when required

Participate in discharge planning and ensure patients are well-informed about post-discharge care

Provide education and emotional support to patients and their families

Adhere to sterile processing protocols to maintain infection control standards

Collaborate with care teams to enhance treatment strategies, particularly for patients with cognitive or memory impairments

Monitor and record vital signs, and assist with patient ventilation as needed

Accurately document all patient interactions and care in medical records
Qualifications and Experience

Clinical experience in a healthcare or medical office setting preferred

Solid understanding of physiology and medical terminology

Familiarity with MDS (Minimum Data Set) assessments is a plus

Experience in sterile processing procedures is highly valued

Previous involvement in discharge planning is an asset

Strong clinical assessment skills with the ability to recognize changes in patient conditions promptly
