Our physician-led care teams include advanced practice professionals and registered dietitians, delivering high-quality care to patients nationwide. Founded in 2019, Form Health is a venture-backed startup dedicated to empowering patients and transforming obesity care at scale, leveraging technology to enhance the patient experience via video visits, messaging, photo journaling, and customized educational resources.
About the Role
Form Health is actively building a talent pool of full-time obesity medicine physicians for anticipated openings in 2025. This role focuses on longitudinal telehealth obesity medicine and medical weight management, collaborating with a multi-disciplinary team to provide high-quality, evidence-based care. Physicians must be licensed in Florida, with additional licensure preferred in New York, Massachusetts, Illinois, and/or Texas.
Key Responsibilities:

Deliver longitudinal telemedicine care for obesity and weight management patients.

Collaborate closely with advanced practice professionals and registered dietitians to ensure coordinated, team-based care.

Conduct patient visits via telehealth platforms, maintaining excellent “web-side” manner.

Provide evidence-based obesity management and guidance tailored to individual patient needs.

Engage in continuous professional development and contribute to the improvement of clinical care processes.
Qualifications

MD or DO degree
-
Board Certified in Obesity Medicine (ABOM)

2+ years of obesity medicine / medical weight management experience
-
Active medical license in FL, with additional multi-state licensure strongly preferred (NY, MA, IL, TX)

Experience conducting telemedicine visits with excellent patient communication skills

Proven ability to work collaboratively with a multi-disciplinary team

Bilingual (Spanish) candidates highly preferred
