Position Summary

We are seeking a skilled Occupational Therapist (Per Visit) to join our Home Health Care team in Eastern Long Island. In this role, you will provide safe, therapeutic, and age-appropriate occupational therapy to help patients regain functional abilities, adapt their surroundings, and improve their daily living skills (ADL/IADL).

Key Responsibilities

Assess & Develop Treatment Plans – Conduct evaluations, set functional goals, and implement interventions.
Modify & Optimize Care – Adjust treatments based on patient progress, home environment, and safety factors.
Collaborate with Healthcare Team – Work closely with interdisciplinary professionals to achieve patient care goals.
Educate Patients & Families – Provide home exercise programs, safety instructions, and functional status updates.
Maintain Accurate Documentation – Ensure compliance with Federal, DOH, and Joint Commission standards.
Enhance Quality & Outcomes – Support performance improvement initiatives and patient satisfaction efforts.

Qualifications

Required:
Bachelor’s Degree in Occupational Therapy (ACOTE-approved NYS program)
Valid NYS Occupational Therapy License & Registration
Basic Life Support (BLS) Certification
1–2 years experience (Hospital, Orthopedic, Outpatient, or Pediatric Rehab)
✔ Strong communication, critical thinking, and customer service skills

Preferred:
Home Care Experience (1–2 years)

Compensation & Benefits

💰 $65.00 per hour (excluding incentives, differentials, and benefits)
