Responsibilities

The Home Health Occupational Therapist (OT) plays a key role in evaluating and addressing patients’ needs related to functional status, activities of daily living, fine motor skills, home safety, adaptive equipment, and other occupational therapy interventions as determined by their medical condition.

Delivers care within the scope of practice as outlined by state regulations and professional guidelines, adhering to the plan of care and utilizing evidence-based methods in collaboration with the healthcare team.
Assesses the patient’s functional abilities, overall health status (as required for CMS documentation), and specific occupational therapy needs. Develops the therapy plan of care in consultation with the physician.
Monitors, documents, and reports the patient’s progress and any changes in condition to the supervising nurse and physician.
Conducts thorough evaluations, utilizing diagnostic and functional ability tests to determine the patient’s level of independence and rehabilitation potential.

Education and Experience

Licensure Requirements:

Current Occupational Therapy license in the state of practice.
Valid CPR certification.
Current driver’s license, proof of auto insurance, and reliable transportation or access to public transit.
