Responsibilities

As a Home Health Occupational Therapist, you will play a key role in assessing and evaluating patient needs related to functional status, activities of daily living, fine motor coordination, home safety, and adaptive equipment. Your goal is to provide personalized, evidence-based care that enhances the patient’s independence and quality of life.

  • Deliver services within the scope of practice as outlined by state laws governing Occupational Therapy, in alignment with the plan of care, and in collaboration with the healthcare team.
  • Assess the patient’s functional abilities, evaluate all body systems as required for CMS documentation, and develop individualized therapy plans in consultation with the physician.
  • Monitor, document, and report patient progress to the nurse supervisor and physician, adjusting treatment plans as necessary.
  • Conduct comprehensive assessments to determine functional capabilities, utilizing diagnostic and prognostic functional ability tests to guide therapy interventions.

Education & Experience

License & Certification Requirements

  • Active Occupational Therapy licensure in the state of Illinois
  • Current CPR certification
  • Valid driver’s license, vehicle insurance, and reliable transportation (or access to public transit)
