Job Summary:
As a Home Health Occupational Therapist, you will assess, plan, implement, and evaluate skilled occupational therapy care for patients in their homes, in collaboration with their families and the interdisciplinary team, as ordered by a physician. You will be responsible for achieving positive clinical outcomes and meeting industry benchmarks.
Primary Duties:
- Provide and coordinate skilled intermittent occupational therapy care, including patient and caregiver education.
- Regularly evaluate and re-evaluate patient status.
- Develop, implement, and revise individualized plans of care.
- Address all identified patient needs and problems.
- Communicate effectively with the interdisciplinary team.
- Report changes in patient condition to the physician and case manager.
- Reassess patients following changes in condition or hospital stays.
Required Qualifications:
- Current Washington State OT license.
- Recent applicable clinical experience in a medical/surgical, rehab, or home health setting.
- Current Washington State Driver’s license and proof of insurability.
- Current Healthcare Provider BLS certification.
- Reliable transportation.
Desired Qualifications:
- Bachelor’s, Master’s, or Doctorate in Occupational Therapy.
