Key Responsibilities:

  • Direct Patient Care: Deliver nursing care to ensure the health and comfort of individuals.

  • Medication Management: Administer medications and maintain precise medical documentation.

  • Daily Support: Assist with daily living activities and implement health-related programs.

  • Medical Coordination: Accompany individuals to medical appointments and provide staff training on health maintenance.

  • Safety & Compliance: Uphold a safe environment by adhering to all relevant regulatory guidelines.

  • Team Collaboration: Partner with healthcare teams, staff, and families to support and follow care plans.


Qualifications:

  • Graduate from an accredited nursing program.

  • Current, active LPN license in New Jersey.

  • Minimum of one year experience working with individuals with intellectual and developmental disabilities preferred.

  • Proficient with basic computer skills and electronic medical record systems.

  • Excellent communication skills, ability to work independently, and a compassionate approach to care.

  • Valid, unrestricted driver’s license with a clean driving record.
