Note: This position is largely remote with occasional travel to provider offices in the Miami-Dade area.


What You’ll Do

As a Patient Engagement Associate II, you will work with your engagement pod to enroll and onboard children and caregivers into Imagine Pediatrics’ program. This highly specialized role requires strong critical thinking, problem-solving, and collaboration skills.

Your responsibilities include:

  • Conduct research on patients to understand healthcare needs, identify barriers, and develop outreach strategies.

  • Perform telephonic outreach to caregivers to enroll children and ensure seamless onboarding.

  • Communicate with healthcare professionals (DME companies, home health providers, pharmacies, pediatricians, etc.) to support enrollment.

  • Leverage provider relationships to re-engage patients.

  • Handle inbound calls from caregivers, health plan coordinators, and hospital discharge planners.

  • Develop knowledge of the Supplemental Security Income (SSI) program and assist caregivers with eligibility outreach.

  • Collaborate with Provider Engagement Specialists for embedded outreach (e.g., PCP offices, residential facilities).


What You Bring

  • 2+ years of healthcare experience in care coordination, community health, patient engagement, customer service, or sales.

  • Proven success in a centralized contact center environment.

  • Strong documentation skills; CRM experience a plus.

  • Excellent collaboration, listening, and communication skills.

  • Ability to tailor messaging to different audiences and overcome objections.

  • Proven relationship-building skills with caregivers and healthcare organizations.

  • Passion for research, data mining, and creative problem-solving.

  • Ability to work independently with high accountability.

  • Access to reliable transportation; willingness to conduct in-field outreach as needed.

  • Bilingual Spanish strongly preferred.
