Position Summary

The Staff Pharmacist plays a key role in supporting the daily operations of the Pharmacy Department, including the preparation and dispensing of medications for patients. This role may involve inventory purchasing—ensuring essential medications, including controlled substances, are available. When a formulary exists, adherence is expected, with exceptions handled according to established procedures.


Key Responsibilities

Medication Preparation & Dispensing

  • Accurately interprets and verifies medication orders and prescriptions (verbal and written), entering them into the electronic medication profile system.

  • Prepares and dispenses medications per licensed provider orders, following all policies and procedures.

  • Compounds sterile preparations, chemotherapy, and parenteral nutrition according to required techniques.

  • Dispenses controlled substances and maintains legally required documentation.

Clinical Oversight & Drug Therapy Monitoring

  • Reviews drug therapies for potential contraindications, interactions, allergies, dosing accuracy, and duplications.

  • Participates in pharmacokinetics consults and supports dose adjustments based on clinical protocols.

  • Promotes cost-effective medication use through therapeutic substitutions and formulary compliance.

  • Supports Antimicrobial Stewardship by reviewing culture and sensitivity data, recommending appropriate therapies.

  • Advises providers on drug alternatives and documents all interventions accurately.

Consultation & Patient Support

  • Provides drug-related information and education to patients, families, and healthcare professionals.

  • Maintains complete and accurate documentation of all clinical activities.

  • Assists in patient medication histories and communicates relevant information to providers.

Departmental Contributions

  • Supervises pharmacy technicians and verifies daily tasks, including ADC (Automated Dispensing Cabinet) operations.

  • Participates in special projects and quality assurance activities such as medication area inspections, crash cart reviews, and safety reporting.

  • Maintains a clean, organized, and compliant pharmacy environment.

  • Assumes pharmacy director responsibilities in their absence when needed.


Professional Competency & Compliance

  • Maintains an active pharmacist license in the State of Texas.

  • Participates in staff meetings, continuing education, and professional development programs.

  • Complies with all hospital, departmental, and regulatory standards and policies.

  • Respects individual diversity in interactions with patients, coworkers, and visitors.


Required Knowledge, Skills, and Qualifications

Education:

  • Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD) from an ACPE-accredited College of Pharmacy.

Licensure:

  • Current and valid pharmacist license issued by the Texas State Board of Pharmacy.

Experience:

  • Previous experience in a hospital or health-system pharmacy is preferred but not required.
