Key Responsibilities

Medication Order Review & Processing

  • Assess medication orders for accuracy, patient-specific factors (weight, allergies, compatibility), and clinical appropriateness.

  • Accurately document and enter orders into the patient profile and ensure timely preparation for delivery.

  • Identify and resolve order discrepancies through collaboration with physicians and nurses; communicate updates to the next pharmacist shift.

Intravenous (IV) Medication Preparation

  • Prepare IV medications including TPN, antibiotics, chemotherapy, and respiratory therapy syringes following all departmental guidelines.

  • Oversee and support training of pharmacy technicians and fellow pharmacists in IV and unit dose procedures.

  • Perform quality checks and ensure accurate, on-time delivery of preparations.

Chemotherapy Handling

  • Process chemotherapy orders with double-checking by two pharmacists for safety and accuracy.

  • Adhere strictly to safety protocols, including the use of PPE (gowns, gloves, mats) and certified hoods during preparation.

  • Calculate doses as specified on chemotherapy patient cardex forms, ensuring utmost precision.

Clinical Consultation & Collaboration

  • Provide expert drug information using current resources to address questions regarding compatibility, dosing, and therapeutic recommendations.

  • Assist physicians with serum concentration timing and drug monitoring.

  • Act as a key clinical liaison at medication windows and via phone, maintaining professionalism and offering proactive solutions.

Leadership & Support

  • Serve as a resource throughout the central pharmacy and assist in areas of need.

  • Act autonomously to address urgent matters in the absence of supervisory staff.

  • Contribute to a culture of collaboration, safety, and continuous improvement within the pharmacy and broader care team.


Qualifications

  • Education: Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD) degree

  • Licensure: Current Pharmacist license in the State of Ohio, or eligibility to obtain within 3 months of hire
