Duties and Responsibilities:

  • Maintain a strong knowledge of sterile IV admixtures, aseptic techniques, and environmental monitoring in compliance with USP 797/800 guidelines.

  • Develop, design, and monitor individualized care plans tailored to the specific medical needs of each patient.

  • Gather relevant medical information from caregivers, case managers, physicians, and patients to determine eligibility for home care services, following One Infusion Pharmacy policies and pharmacy practice guidelines.

  • Demonstrate proficiency in therapeutic drug levels and common laboratory values, and perform advanced pharmaceutical calculations.

  • Monitor and track lab values related to drug regimens, offering informed pharmaceutical recommendations and making adjustments as needed to optimize patient care.

  • Serve as the subject matter expert in pharmacotherapy, educating other One Infusion staff (including nurses and pharmacy technicians) and patients/caregivers.

  • Oversee the activities of ancillary pharmacy staff to ensure compliance with pharmacy protocols and physician orders.

  • Contribute to continuity of care across settings, ensuring accurate preparation and dispensing of medications according to USP 797/800 and company policies.

  • Conduct quality control checks on final preparations before delivery, ensuring accuracy in concentration, quantity, stability, and labeling.

  • Ensure compliance with all applicable laws and regulations (e.g., DEA, BOP, ACHC) to meet or exceed accreditation standards.

  • Address and resolve patient concerns and provide counseling on medication use as needed.

  • Monitor and report medication errors, adverse drug reactions, allergies, and patient adherence issues, and perform patient profile monitoring as necessary.

  • Collaborate with patients, caregivers, and healthcare professionals, providing updates on patient status and medication responses while documenting interventions promptly.

  • Report any misconduct or unethical activities to the Compliance Officer or supervisor.

  • Interact with pharmaceutical representatives to stay informed about pharmaceutical products and manage formulary changes.

  • Monitor inventory levels to ensure adequate supply, manage market shortages and product recalls, and maintain perpetual inventory of controlled substances as required.

  • Assume on-call responsibilities per pharmacy guidelines and company policies.

  • Report product incidents according to pharmacy policy and collaborate with the Performance Improvement Coordinator.

  • Participate in the pharmacy’s Performance Improvement program as needed.

  • Engage in pharmacy committees, in-service education programs, and professional organizations as directed by the Pharmacy Director.

  • Complete continuing education programs as required to maintain licensure.

  • Perform other duties as assigned by the supervisor.

Regulatory Requirements:

  • Graduate from an accredited School of Pharmacy.

  • Hold a current, active pharmacist license (Pharm.D., Rph) in the state of practice.

  • Demonstrate a strong knowledge of sterile IV admixtures and USP 797/800 guidelines.

  • Minimum of 1-2 years of clinical pharmacy experience in home care, hospital, or infusion settings.

  • Experience with the CPR+ computer program.

  • Completion of an ASHP-accredited Pharmacy Residency Program or Advanced Pharmacy Practice Experience with an accredited school of pharmacy.

  • At least 2 years of clinical experience in home care, hospital, or infusion settings.

Physical Requirements:

  • Ability to engage in physical activity, including lifting and pushing up to 50 pounds without restriction.

  • Ability to stand for extended periods and perform physical tasks, including bending and lifting regularly.

Language Skills:

  • Proficient in English, both verbally and in writing.

  • Additional languages are a plus.

Scheduled Weekly Hours:

  • 1 hour.

Pay Range:

  • $104,000 – $143,000 per year, based on geographic location, skills, experience, education, certifications, and other factors.
