Responsibilities

Serve as a clinical leader for pharmacy services within a designated area of specialization, guiding best practices and pharmaceutical standards.

Collaborate with physicians and interdisciplinary care teams to develop strategies that optimize medication use, including creating clinical pathways, medication guidelines, and educational policies.

Provide direct patient care through collaborative pharmaceutical management, ensuring safe and effective use of medications in specialized clinical settings.

Administer injectable medications, evaluate ongoing drug therapies for safety, efficacy, and cost-effectiveness, and document all interventions in the Electronic Health Record (EHR).

Lead initiatives to enhance pharmaceutical care by conducting drug utilization reviews, developing performance metrics, and supporting departmental and system-wide goals.

Ensure compliance with all regulatory standards related to pharmacy practice within the area of specialization.

Participate in continuous quality improvement activities, helping to identify and address gaps in patient care.

Contribute clinical expertise to multidisciplinary committees, support evidence-based decision-making, and assist in the development of standardized protocols and Quality Assurance/Performance Improvement (QAPI) programs.
Required Education, Experience & Skills

Bachelor’s or Doctorate degree in Pharmacy (BSPharm or Pharm.D.)

Completion of a PGY1 and PGY2 residency in a relevant specialty, OR PGY1 plus two years of specialty practice experience, OR a minimum of five years of experience in the area of specialization.
Required Licensure & Certification

Active California Pharmacist License

American Heart Association (AHA) Basic Life Support (BLS) for Healthcare Providers

Board of Pharmacy Specialties (BPS) or other nationally accredited certification required within two years of hire
Preferred Qualifications

Experience in managing care in an ambulatory patient care setting

Proficiency with the Epic EHR system in an ambulatory environment
