Your Responsibilities:

• Conduct thorough patient evaluations and develop individualized treatment plans

• Continuously monitor and reassess treatment effectiveness, participate in team staffing meetings and family conferences

• Complete discharge evaluations, provide recommendations for home and community safety, and offer referrals and educational resources as needed

• Maintain clear communication regarding patient progress and needs with the treatment team, physicians, patients, and family members

• Collaborate with other healthcare disciplines to ensure coordinated care and optimize patient outcomes

Qualifications Required:

• Bachelor’s degree or higher in Physical Therapy from an accredited program

• Current and unrestricted physical therapy license in the state of practice

• Valid Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification as required by state regulations

• Preferred: At least one year of clinical experience as a Physical Therapist

• Demonstrated expertise in rehabilitation techniques for complex neurological injuries
