About the Role
Acacia Health Center at Sagewood is seeking a dedicated Physical Therapist to join our team. In this role, you will provide direct patient care, oversee treatment planning, and ensure the highest standards of wellness for residents. Responsibilities

Maintain accurate records of patient care and billing information.

Provide direct patient care, evaluations, and treatment planning.

Report and interpret treatment results.

Supervise support personnel as appropriate.

Conduct screenings, develop treatment plans, and deliver therapy in line with facility and Medicare guidelines.

Assist with functional mobility, transfers, gait, and balance.

Participate in facility meetings, in-services, and community education activities.

Provide education to patients, families, and staff regarding therapy goals and treatment plans.

Collaborate with staff to ensure consistent, quality patient care.
Qualifications
-
Doctorate in Physical Therapy (required).

Licensed by the State in respective therapy profession.
-
CPR certification.
-
Ability to perform 90% physical tasks, with 10% sedentary tasks.
