As a Home Health Physical Therapist, Your Responsibilities Will Include:

  • Deliver prescribed, skilled physical therapy treatments to patients managing a variety of injuries, illnesses, and physical impairments, helping them achieve their highest level of function.
  • Conduct thorough evaluations and physical assessments to assist physicians in determining patient functionality and document findings to support treatment planning.
  • Design and implement individualized rehabilitation and conditioning programs in alignment with the physician’s Plan of Treatment and the patient’s overall goals, modifying therapy as necessary to maximize outcomes.
  • Collaborate with physicians and interdisciplinary clinical teams to gather patient information and contribute to the development, implementation, and adjustment of therapy plans.
  • Provide clear, written care plans and instructions to Physical Therapy Assistants and Home Health Aides, supervise their performance, and ensure accurate documentation of patient care.
  • Promptly and thoroughly document all patient interactions, interventions, and progress. Ensure verbal orders from physicians are accurately recorded, submitted for signature, and carried out as directed.
  • Communicate patient progress regularly to the physician, Clinical Manager, care team, patient, and family. Submit evaluations, care plans, progress updates, and discharge summaries to leadership and care management teams.

Required Experience & Qualifications:

  • A degree from an accredited Physical Therapy Program (APTA-approved).
  • A minimum of one year of experience in physical therapy preferred.
  • A current and unrestricted Physical Therapy license.
  • Valid CPR certification.
  • Strong organizational, interpersonal, and communication skills.

Compensation Range:

This represents a good faith estimate for full-time (40 hours per week) employment at the time of posting. Actual compensation may vary depending on location and individual qualifications such as skills, experience, education, and certifications.

  • $54.00 – $76.00 per visit/unit
  • $85,400 – $117,500 annual base salary
