As a Home Health Physical Therapist, your responsibilities will include:

  • Planning and delivering prescribed physical therapy treatments for patients dealing with injuries, illnesses, or functional disabilities, aiming to help them achieve the highest level of physical function.

  • Assessing the patient’s physical strengths to support the physician in evaluating their functional status and documenting findings to create or adjust treatment programs and measurable goals.

  • Developing and implementing rehabilitation programs in line with the physician’s treatment plan and the overall goals of the patient and rehab team. Treatment adjustments will be made as needed to achieve optimal outcomes.

  • Collaborating with physicians and the clinical team to gather additional patient information and refine therapy treatment plans.

  • Providing written instructions and care plans to Physical Therapy Assistants and Home Health Aides, ensuring they reflect the current therapy plan, while supervising and evaluating their performance.

  • Ensuring the accurate, timely, and thorough documentation of patient care, including progress reports, interventions, and evaluations. Verbal orders from the physician and physical therapist should be promptly documented, submitted for physician approval, and implemented.

  • Reporting patient progress to the physician, Clinical Manager, staff, patients, and families, submitting evaluation, treatment plans, progress reports, and discharge summaries.

Required Qualifications:

  • Degree from an accredited Physical Therapy Program approved by the APTA.

  • At least one year of physical therapy experience is preferred.

  • A current, unrestricted Physical Therapy license.

  • Current CPR certification.

  • Strong organizational and communication skills.

Pay Range:

  • $64.00 – $89.00 per visit/unit.

  • $93,900 – $129,300 annually (base pay).
