Role Overview:
We are seeking a dedicated Physical Therapist to provide compassionate, high-quality care to our home health patients across multiple counties in Florida. As a valued member of our team, you will deliver skilled physical therapy services designed to alleviate pain, restore function, and maximize patient performance within the guidelines of individualized treatment plans.
Key Responsibilities:

Administer physical therapy treatments in accordance with prescribed treatment plans and clinical protocols.

Provide therapeutic interventions aimed at relieving pain, enhancing mobility, restoring function, and maintaining optimal patient performance.

Continuously monitor patient progress and adjust treatment approaches as necessary, following physician directives.

Communicate assessment results and treatment progress effectively to patients, their families, and the multidisciplinary care team.

Ensure compliance with all regulatory guidelines and documentation standards.
Required Qualifications and Skills:

Current and valid Florida State Physical Therapy license.

Valid driver’s license with reliable transportation for home visits.

Active CPR certification.

Minimum of one year of verifiable physical therapy experience; prior home health experience is highly preferred.

Strong communication and interpersonal skills to engage effectively with patients, families, and colleagues.
