You’ll deliver personalized rehabilitation services focused on functional improvement, empowering patients to overcome physical challenges, enhance their quality of life, and return to the activities they enjoy.
Your Role as a Home Health Physical Therapist
As a Physical Therapist, you will:

Design and administer skilled physical therapy interventions for patients with injuries, illnesses, or physical limitations.

Assess patients’ physical capabilities to aid in physician evaluations and establish measurable rehabilitation goals.

Develop and adjust therapy plans aligned with the physician’s orders and the patient’s overall health goals.

Collaborate closely with physicians and care team members to evaluate progress and modify treatment plans as needed.

Provide written care instructions to Physical Therapy Assistants and Home Health Aides and supervise their performance.

Ensure accurate and timely documentation of all patient assessments, treatments, progress notes, and verbal physician orders.

Regularly update physicians, clinical managers, patients, and families on therapy progress.

Submit comprehensive reports including evaluations, care plans, and discharge summaries to clinical supervisors.
Qualifications and Requirements

Degree from an accredited Physical Therapy program (recognized by APTA).

Current and unrestricted Physical Therapy license.
-
CPR certification.

Valid driver’s license, auto insurance, and reliable transportation.

At least one year of professional experience preferred.

Strong communication and organizational skills.
Compensation & Schedule
-
Hourly Pay (per visit/unit): $54.00 – $76.00
-
Annual Base Pay Range: $85,400 – $117,500
-
Scheduled Weekly Hours: 40
Final compensation is determined based on location, experience, qualifications, and applicable laws.
