Home Health Physical Therapist
What You’ll Do
As a Home Health Physical Therapist, you’ll play a vital role in helping patients recover from injury, illness, or surgery and manage chronic conditions—all from the comfort of their own homes. You’ll develop and deliver customized therapy plans aimed at restoring physical function and improving quality of life.
Key Responsibilities

Administer prescribed physical therapy treatments to help patients regain strength, mobility, and independence.

Evaluate patients’ physical abilities to support the physician in determining functional status and setting measurable goals.

Create and implement individualized rehabilitation and conditioning programs aligned with the physician’s treatment plan and patient goals.

Modify treatment strategies as needed to achieve the best possible outcomes.

Collaborate with physicians and interdisciplinary care teams to review and revise patient care plans based on ongoing assessments.

Provide detailed care instructions and supervision to Physical Therapy Assistants and Home Health Aides, ensuring care plans are followed accurately.

Oversee and review therapy documentation by PT Assistants and Aides to ensure compliance and continuity of care.

Maintain thorough and timely records of patient observations, interventions, and evaluations. Ensure all verbal orders are promptly documented, submitted for physician signature, and carried out.

Regularly communicate patient progress to physicians, clinical managers, team members, patients, and families. Submit evaluations, progress notes, care plans, and discharge summaries to the appropriate care team staff.
Required Qualifications

Graduate of an accredited Physical Therapy program (APTA-approved)

Current, unrestricted Physical Therapy license

At least one year of experience in physical therapy (preferred)

CPR certification (current)

Excellent communication and organizational skills

Valid driver’s license, reliable transportation, and active auto insurance
Schedule

Full-time (40 hours per week)
Compensation
The starting salary range for this full-time role is estimated between $84,600 and $116,300 annually, depending on geographic location, education, experience, skills, and certifications.
