Job Summary
Kaiser Permanente is seeking a compassionate and skilled Physical Therapist to join our team at the Fair Oaks Medical Center in Fairfax, VA. In this role, you will provide comprehensive therapy services under the direction of the Rehabilitation Manager. You will evaluate patients, create and implement treatment plans, and guide members toward improved mobility, function, and independence.

Responsibilities
As a Physical Therapist, you will conduct patient assessments to identify impairments, functional limitations, and participation barriers. You will then design and deliver individualized treatment programs that may include in-person, telephonic, or video-based care. Active collaboration with patients, families, and caregivers is essential to ensure informed consent and engagement throughout the therapy process.
You will educate patients on home exercise programs, monitor progress, and adjust treatment plans as needed. Accurate documentation and communication with medical providers are critical, as is the supervision of Physical Therapist Assistants in accordance with jurisdictional requirements.
In addition to direct patient care, you will help maintain a safe and well-equipped clinical environment, uphold professional and ethical standards, and contribute to program development, staff mentoring, and patient education initiatives. At times, you may provide coverage at other facilities within the region to support continuity of care.
Qualifications
Education

Graduate of an accredited physical therapy program (bachelor’s, master’s, or doctoral degree) or foreign-trained equivalent.
Licenses & Certifications
-
Physical Therapist License in Maryland, Virginia, and the District of Columbia OR Compact License (within 6 months of hire).

Virginia Direct Access Certification (within 6 months of hire).

Basic Life Support (BLS) certification from the American Heart Association.

PECOS Enrollment (within 1 month of hire).

National Provider Identifier (NPI).
Additional Requirements

Ongoing pursuit of advanced education, fellowship, certification, or clinical expertise relevant to patient care.

Demonstrated leadership skills, problem-solving ability, and professional practice.

Experience in mentoring, staff development, clinical teaching, project management, or committee participation.
Preferred Qualifications
-
Experience teaching physical therapy–related health education classes.

Involvement in supervising and orienting volunteers.

Participation in at least two community health events annually, such as classes, health fairs, or medical center functions.

Competency in at least 50% of PT clinical specialties.
