Legacy Healthcare Services specializes in delivering Physical Therapy, Occupational Therapy, Speech-Language Pathology, and Wellness services exclusively to the senior population. We serve residents in Skilled Nursing Facilities, Continuing Care Retirement Communities (CCRCs), and stand-alone Assisted and Independent Living communities.
Headquartered in Raleigh, North Carolina, Legacy currently operates across 16 states and Washington, D.C. Our services are provided through contract arrangements with skilled nursing and CCRC facilities, as well as through our many Medicare-certified outpatient clinics located within senior living communities.
Our clinical approach blends rehabilitation with healthy aging practices, promoting independence in both home and community settings. Legacy offers both rehabilitative and preventative care, with a strong focus on health education and fitness to empower seniors in maintaining an active, independent lifestyle.
We’ve developed a variety of specialized clinical programs to help seniors live their best lives. Our team is highly skilled in managing all aspects of skilled therapy for patients during their SNF Part A stay. Additionally, we understand the value of on-site therapy services within Assisted and Independent Living communities—offering residents convenient access to high-quality care in a familiar setting.
This consistent presence fosters trust and enables our therapists to detect subtle changes in a resident’s condition early—changes that, if left unaddressed, could lead to reduced mobility, social isolation, or accelerated decline. Our therapists can recommend follow-up care through on-site wellness or activity programs and continue to monitor each individual’s progress, helping ensure lasting improvements and enhanced quality of life.

Qualifications

Associate’s Degree in Applied Science in Physical Therapist Assisting or related discipline

Active license (or eligibility to obtain) in the state of practice

Strong clinical, interpersonal, and time-management skills
-
Comfortable using computers and electronic documentation systems
