Our team is highly skilled in managing all aspects of skilled therapy for patients during their SNF Part A stay. This consistent presence fosters trust and enables our therapists to detect subtle changes in a resident’s condition early—changes that, if left unaddressed, could lead to reduced mobility, social isolation, or accelerated decline. Our therapists can recommend follow-up care through on-site wellness or activity programs and continue to monitor each individual’s progress, helping ensure lasting improvements and enhanced quality of life.

Qualifications

  • Associate’s Degree in Applied Science in Physical Therapist Assisting or related discipline

  • Active license (or eligibility to obtain) in the state of practice

  • Strong clinical, interpersonal, and time-management skills

  • Comfortable using computers and electronic documentation systems
